Please fill out a new registration form for each student.
SPRING 2021 OPEN REGISTRATION:
No child will be placed on a roster until:
The registration form is completed;
The appropriate fees are remitted ; and
A ++Physician’s Release Form++ is signed by your child’s doctor with the date of the physical. It is good for 12 months from date of physical, please do not submit a physical exam.
