As our athletic programming grows and expands, we will be utilizing Final Forms to collect all student information, physicals, and parent supplied contact information.
This platform will be used to collect payments only. This includes, but not limited to: participation fees, uniform supplemental pieces, and optional add on purchases.
Most sports are open to current OFMS and non-OFMS students.
Please note the following tips to completing the registration process!
Selection - Please review all the options and add team selections to your cart. In some instances, if you are a returning player, your requirements may be different.
Team Gear - When your team selections are complete, please CONTINUE to the Team Gear section to purchase required and optional Oak Farm Falcon gear associated with your sport.
Payment - All registrations/payment will take place online. We will not be accepting paper registrations or cash/checks. Registration is not complete until payment is made.
