Parents will use this form to register for the sacrament of Confirmation at St. Thomas the Apostle Parish taking place in the fall of 2024.
NOTES:
NON-PARISHIONERS -
If you are registered in another parish but wish your child to make the sacrament of Confirmation at St. Thomas, you MUST have WRITTEN permission from the parish where you are registered. This must be received in the PREP office in order for your child to receive the sacrament. Please provide this as soon as possible and no later than June 15, 2024.
SPONSOR SELECTION
All sponsors need letters of eligibility (including St. Thomas parishioners). Sponsors must forward their letter to our office as soon as possible and no later than June 15, 2024.
Choose a sponsor who:
is at least 16 years old
has received the sacraments of Baptism, Holy Communion and Confirmation
if married, must have been married in the Catholic Church
is a fully active and practicing Catholic who fulfills the weekly obligation to attend Mass.
is a good example in living a Caholic life that reflects the teaching and spirit of the Caholic Church.
is prepared to assist the parents in their Christian duty by support, encouragement and prayers.
ROBE INFORMATION:
You may purchase a robe below for $25, or select NO ROBE if you will not be purchasing a robe. For those ordering robes, we will send out a form in August to specify the candidate's height.
Candidate may wear a robe from an older sibling or friend from a previous St. Thomas confirmation, as long as it fits properly (should reach the shins in length).
The robe fee can be paid by credit card or e-check (processing fees apply) or by cash/check delivered to our office. Robe sizes will be measured at school/PREP in the fall and delivered to our office.
Please complete a separate application for each child who will be receiving the sacrament if you have more than one.
